From I-40 in Jackson, take U.S. 45 north. At Three Way, U.S. 45 North divides into 45W and 45E. Take U.S. 45W and continue through Humboldt. Trenton and Dyer. Dyer Nursing & Rehabilitation Center is on the left, just north of the Dyer business district.
